黃帝曰:愿聞脹形。岐伯日:夫心脹者，煩心短氣，臥不安。肺脹者，虛滿而喘咳。肝 脹者，脅下滿而痛引小腹。脾脹者，善噦，四肢煩愧，體重不能勝衣，臥不安[臥不安:按此三字是衍文]。腎脹者，腹?jié)M引背央央然，腰髀痛。六腑脹:胃脹者，腹?jié)M，胃脘痛，鼻聞焦臭，妨于食，大便難。大腸脹者，腸鳴而痛濯濯[濯濯:《甲乙》卷六第八并無此二字],冬日重感于寒，則飧泄不化。小腸脹者，少腹滇脹，引腰[腰:《脈經(jīng)》卷六第四作“腹”]而痛，膀胱脹者，少腹?jié)M而氣癃。三焦脹者，氣滿于皮膚中，輕輕然而不堅(jiān)。膽脹者，脅下痛脹，口中苦，善太息。凡此諸脹者，其道在一，明知逆順，針數(shù)不失。瀉虛補(bǔ)實(shí)，神去其室，致邪失正，真不可定，粗之所敗，謂之天命。補(bǔ)虛瀉實(shí)，神歸其室，久塞其空，謂之良工。

Yellow Emperor said: "I hope to hear about the appearances of the syndrome of distention." Qibo said: "In the distention related to the heart, the patient will have oppression in the chest, short or breath, restless when sleeping; in the lung distention, the patient has oppression of deficiency type in the chest, rapid respiration and cough; in the distention due to liver disorder, the patient will have distention and fullness under the hypochondria and drawing pain in the lower abdomen; in the spleen-distention, the patient will hiccup often, his four extremities will be clumpsy and his body will be feeling heavy and will not likely to withstand wearing clothes; in the kidney-distention, the patient will have fullness in the abdomen which drawing the back to become uncomfortable, and the loins, joints of extremities to become painful. As to the distention of the six hollow organs: the stomach-distention will cause fullness of the abdomen and pain of the gastric cavity, the nose seems to have smelt the scorching odour which obstruct eating, and the patient will have dyschesia: in the flatulence of large intestine, the patient will have borborygmus and pain in the intestine, when catching cold in winter, he will have diarrhea with indigested food; in the flatulence of the small intestine, the patient will have distention and fullness in the lower abdomen and pain in the abdomen: in the distention of the urinary bladder, the patient will have distention and fullness in the lower abdomen and the retention of urine; in the triple warmer-distention, the patient will have plenty of air in the skin to cause swelling, and the skin is empty and soft when pressing: in the distending pain of the hypochondrium due to galibladder disorder, the patient will have pain under the hypochondrium, bitter taste in the mouth and he sighs often. The treating principle to the various diseases of distention stated above are the same. The treating will be proper when one knows clearly the agreeable and adverse relations with the disease and when the times of pricking (one or three times) are correct. If purging therapy is applied to asthenic syndrome or the invigorating therapy is applied to the sthenic syndrome, the spirit of the patient will be dispersed, the evil energy will penetrate to injure the healthy energy to cause unstableness. A physician of lower level often fails to treat in a proper way, and he is supposed to be the one to cause the patient die early; if one applies invigorating therapy to the asthenic syndrome and purging therapy to the sthenic syndrome, the spirit of the patient will be maintained inside, his orifices will be stable and filling with healthy energy, the physician is supposed to be a good one."

黃帝日:脹者焉生?何因而有[而有:《太素》卷二十九《脹論》“有”下并有“名”字]?岐伯日:衛(wèi)氣之在身也，常然[然:《太素》卷二十九《脹論》并無此字]并脈循分肉，行有逆順，陰陽相隨，乃得天和，五臟更始[更始:《太素》卷二十九《脹論》“始”作“治”]，四時(shí)循序，五谷乃化。然后厥氣在下，營衛(wèi)留止，寒氣逆上，真邪相攻，兩氣相搏，乃合為脹也。黃帝日:善。何以解惑?岐伯日:合之于真，三合而得。帝日:善。

Yellow Emperor asked: "Where does distention generate and why it has the name of distention?" Qibo answered: "The Wei-energy in the body usually runs between the boundaries of muscles along with the channel, if the Wei-energy runs agreeably along the three foot channels and runs adversely along the three hand channels to keep the Yin and Yang harmonious, one will receive the genial energy of nature, the five solid organs will be kept in order, the four seasons will elapse in regular sequence, and food in the stomach will be digested and be absorbed normally, and one will be healthy. If the cold energy is hidden below, retaining between the Ying-enery and the Wei-energy, then, the flowing of both the energies will be abnormal, the cold energy will reverse up, the healthy energy and the evil energy will be combating, and the disease of distention will be shaped." Yellow Emperor said: "Good. But how to remove my perplexion?" Qibo said: "It is correct to apply the purging therapy, and to the protracted disease, the purging of three times is suitable. " Yellow Emperor said: "Good."

黃帝問于岐伯日:脹論言無問虛實(shí)，工在疾瀉，近者一下，遠(yuǎn)者三下。今有其三而不下者，其過焉在?岐伯對日:此言陷于肉育，而中氣穴者也。不中氣穴，則氣內(nèi)閉;針不陷肓，則氣不行;上[上:《太素》卷二十九《脹論》作“不”]越中肉，則衛(wèi)氣相亂，陰陽相逐。其于脹也，當(dāng)瀉不瀉，氣故不下，三而不下，必更其道，氣下乃止，不下復(fù)始，可以萬全，烏有殆者乎。其于脹也，必審其彌[彌:《甲乙》卷八第三并作“診”]，當(dāng)瀉則瀉，當(dāng)補(bǔ)則補(bǔ)，如鼓應(yīng)桴，惡有不下者乎。

Yellow Emperor asked Qibo: "You have said that one can disregard whether the distention is of asthenic or sthenic, but only applies purging by pricking hastily will be effective, pricks once when the disease is newly contracted, and pricks three times when the disease is protracted. Now, I have purged for three times, but the distention has not relieved, where is the error?" Qibo answered: "The so called prick once or prick three times here means the pricking must reach the membrane which is under the skin and above the muscle and one must also hit the acupoint of distention. If the acupoint is not hit, the energy will be shut inside without excreting, when the needle fails to reach the membrane under the skin and above the muscle, the energy will not be running between the boundaries of muscles, when the pricking is not hitting the acupoint but hit the place between the muscles erroneously, the Wei-energy will run rashly and the Yin and Yang energies will be contending inside. When treating distention by pricking, if one fails to purge when it should be purged, the energy of distention will not be dissipated, under this condition, one must prick by changing the acupoint until the energy of distention is removed. If it is not removed, one should prick again, and no accident will occur. When treating distention, one must examine carefully whether the syndrome belongs to the solid organ or to the hollow organ, purge when it should be purged and invigorate when it should be invigorated, then, the curative effect will be as clear as the sound of drumsticks hitting the drum. When one treats like this, how could any distention be not cured?"
